Proper Facility Discharge

We determine the patient's proper facility placement based on their needs before discharge.

Guardianship​

ETG (Emergency Temporary Guardianship). An attorney will file a petition with the local court where the ward resides.

Power of Attorney

An attorney can draft the appropriate Power of Attorney and Healthcare Proxy documents.

ICP Medicaid Application​

We complete the Medicaid application for the patient, which requires gathering financial documents ie; bank statements, 2613 Documents DCF (Financial Form), Social Security Award Letters, etc.
